Stress distribution of customized abutment material and angles in dental implant systems: A finite element study

Aim: This study aims to use finite element analysis to evaluate stress distribution in implant components, restoration materials, and biological tissues for implants placed at different angles using various customized abutment materials. Methodology: 3D models of a three-member implant-supported fixed prosthesis were created using SolidWorks, with implants placed at positions 44 (4I) and 46 (6I). These models were imported into EXOCAD to design prosthetic restorations on custom abutments at three angles: straight (KD0), 10° (KD10), and 20° (KD20). Abutments were made from alumina (AL), titanium (TI), and zirconia (ZI). A 150 N load was applied to the occlusal table at a 30° angle in the buccolingual direction. Stress distribution was measured as von Mises Stress (vMS) values. Results: For implants, the lowest vMS value was observed in the KD0-AL group (4I: 9.984 MPa, 6I: 22.91 MPa), while the highest vMS value was found in the KD20-TI group (4I: 25.30 MPa, 6I: 42.03 MPa). Custom abutment and custom abutment screws (in general) showed higher vMS values in the AL material and lower vMS values in the TI material. In contrast, for cortical bone and restoration material, higher vMS values were observed in the TI material and lower vMS values in the AL material. Conclusion: Dental implants require effective stress distribution from occlusal loads to ensure long-term success. Excessive stress on the abutment and abutment screw can compromise the prosthetic outcome. Therefore, titanium (TI) abutments are preferred in the posterior region for their durability despite being less aesthetically pleasing. Analysis of stress distribution in implant-supported prostheses with custom abutments at different angles of 20 degrees: A finite element analysis

Aim: Implants may not always be placed parallel to each other. In such cases, implants can be made more parallel by adjusting the angles with custom abutments. Different cementation materials can be used for cement-retained abutments. The bonding and stress resistance values of materials used in the cementation of implant-supported prostheses vary. The aim of this study is to demonstrate the stress distribution values of restorations bonded with different cementation materials in finite element analysis (FEA). Methodology: In this study, a maxillary edentulous jawbone was designed using the SolidWorks program. Two implants were placed in the regions of teeth #14 and #16. Implant data were obtained from the Bilimplant company. A 20-degree angled custom abutment and implant-supported prosthesis were designed using the Exocad program. Standard Tesellation Language (STL) files were transferred to SolidWorks to obtain Standard for the Exchange of Product Model Data (STP) files. The created prosthesis design was then transferred to the ABAQUS program for finite element analysis. Subsequently, using five different cementation materials (glass ionomer cement, dual-cure resin cement (Ivoclar), dual-cure resin cement (RelyX), polycarboxylate cement, and self-cure resin cement), stress distribution values of implant-supported prosthesis components were recorded as von Mises stress (VMS) values. Results: The results indicate higher stress values with glass ionomer cement and lower values with polycarboxylate cement. Stress distribution increases posteriorly but does not differ significantly based on cement type. The lowest stress on implant screws was found with glass ionomer, but the highest stress varied. Cortical bone stress is lowest with glass ionomer cement. Conclusion: No significant difference was found in stress values among different cement types. Stress increases in implants, abutments, and cements due to forces applied posteriorly, while anteriorly, retention screw stress increases. The relationship between xerostomia and periodontal status in patients with cardiovascular diseases and type 2 diabetes

Aim: The aim of this cross-sectional study was to examine the correlation between patients' subjective perception of xerostomia and their periodontal status and to examine the potential association of this correlation with cardiometabolic disorders. Methodology: A total of 147 patients (40 systemically healthy, 33 with cardiovascular disease [CVD], 40 with type 2 diabetes, and 34 with cardiometabolic disease [CVD + diabetes]) who came to Necmettin Erbakan University, Faculty of Dentistry, Department of Periodontology for periodontal treatment and agreed to participate were included in the study. Sociodemographic information, tooth brushing frequency, systemic disease status, and clinical periodontal parameters (plaque index [PI], gingival index [GI], bleeding on probing [BOP], and mean pocket depths) were recorded. Patients were asked to complete an 11-item subjective xerostomia inventory questionnaire. Results: In the findings of our study, there was no statistically significant correlation between xerostomia total score and mean pocket depths, PI, GI, and BOP ( p >0.05). There was no statistically significant difference between the mean values of xerostomia total score according to systemic disease groups ( p =0.654). There is a statistically significant difference between the mean values of xerostomia total score according to periodontal status groups ( p =0.002). The mean xerostomia total score was 20.19 in periodontally healthy patients, 24.42 in gingivitis patients, and 22.73 in periodontitis patients. Conclusion: Although the results suggest that subjective dry mouth findings are associated with gingivitis, they do not affect pocket depth and do not increase susceptibility to periodontitis. Clinicians should emphasize the risk of gingivitis in patients with dry mouth and the importance of prevention. Investigation of the effect of the application of different full ceramic materials on stress distribution to minimally invasive posterior fixed bridge restorations by finite element analysis

Aim: The aim of this study was to examine the effect of different types of all-ceramic materials used in various designs of partial posterior indirect restorations on stress distribution through finite element analysis. Methodology: Intact mandibular first premolar and first molar teeth were selected. After being soaked in saline solution for a week, 3D radiographic images were taken using cone-beam computed tomography (CBCT). Following root canal treatment, preparations were made for inlay restorations on both teeth. CBCT images were taken again after the preparations. Data from CBCT scans were used to create 3D models in the Mimics program. Restoration designs were prepared in Standard Tessellation Language (STL) format using the EXOCAD program. The models were transferred to SolidWorks for collision processes with cement designs and then exported in Standard for the Exchange of Product Model Data (STP) format. These files were imported into the ABAQUS program for finite element analysis, with properties assigned for each restoration type. Vertical forces of 600 N were applied to the occlusal surfaces, and stress values were recorded in Von Mises Stress (VMS) values. Results: The results of the study showed that the VMS value on inlay bridge restorations was lower in mesio-occluso-distal inlay (MODI) compared to MO-DOI. ZLS exhibited the lowest VMS value for enamel loads, while nano-hybrid composite ceramics with resin matrix (RMNH) showed the lowest stress distribution in the restoration material. In posterior adhesive indirect bridge restorations, material rigidity reduced tooth load distribution. Thus, MODI preparations are preferable due to reduced stress on teeth. ZLS is advantageous as the restoration material for minimizing enamel stress. Conclusion: In cortical and trabecular bone, similar values were found across all three restorations. Assessment of self-reported symptoms and oral health-related quality of life in patients requiring periodontal treatment

Aim: Oral health-related quality of life (OHRQoL) refers to the impact of social, psychological, and functional factors, as well as problems in the oral-facial region, on individuals’ well-being. This observational study aimed to evaluate the relationship between self-reported symptoms and OHRQoL and the periodontal status of patients presenting for periodontal treatment. Methodology: The study comprised 170 individuals recruited from the Department of Periodontology at Necmettin Erbakan University. Participants were divided into three groups based on their periodontal status: periodontally healthy (65 individuals), gingivitis (51 individuals), and periodontitis (54 individuals). Various clinical periodontal parameters, such as the plaque index (PI), gingival index (GI), bleeding on probing (BoP), tooth mobility, and clinical attachment loss (CAL), were assessed alongside self-reported periodontal symptoms. Additionally, participants completed the Oral Health Impact Profile-14 (OHIP-14) questionnaire. Results: Both the periodontitis and gingivitis groups exhibited similar outcomes concerning gingival redness and gingival bleeding during brushing. However, a significantly higher number of affirmative responses for other symptoms were recorded in the periodontitis group ( p <0.05). Additionally, the periodontitis group had the highest total OHIP-14 scores and subscale scores, while the periodontally healthy group exhibited the lowest scores ( p <0.05). Conclusion: This study demonstrates a significant relationship between OHRQoL, patients' self-reported symptoms, and periodontitis. Comparison of the effect of different rapid maxillary expansion treatments on tooth hardness values: Splint mouth study

Aim: The purpose of this study, planned as a splint mouth design, is to compare the effects of different rapid maxillary expansion (RME) appliances on the hardness values of teeth. Methodology: This study utilized extracted upper first premolar teeth from a previous study conducted by Yıldırım and Akın in 2019 on 20 patients. Five patients were excluded from this study due to fractures of the tooth crown during extraction, which also necessitated the exclusion of the opposing arch teeth. The extracted teeth were stored in distilled water at +4 degrees Celsius. The crowns and roots of these teeth were separated at the cementoenamel junction using an aerator and a diamond bur under water cooling. The crowns were then embedded into rings of standard thickness using the cold acrylic dripping method. Hardness measurements were performed using a Vickers Hardness tester. Enamel samples obtained from the patients were placed under a test microscope and examined at X40 magnification; force was applied to each enamel sample for 10 s. The device automatically calculated and recorded the microhardness value relationship. The Shapiro-Wilk test was used to confirm the normal distribution of the data. Comparisons between groups were conducted using Student's t-test. Results: The group using the hybrid maxillary expansion appliance showed lower hardness values than the conventional group, although the difference was not statistically significant. Conclusion: Fixed orthodontic treatments similarly increase the demineralization of teeth. Evaluation of color stability after the coloring process in bulk-fill composites

Aim: Bulk-fill composites have been developed to overcome the disadvantages encountered in the incremental technique, such as the risk of contamination between layers, bonding problems, and gaps. Bulk-fill composites are frequently used in dentistry. The aim of this study was to evaluate the color stability of three different bulk-fill composites after a staining period with tea, coffee, red wine, and distilled water. Methodology: Three different flowable bulk-fill composites were used in the study (3M Filtek One, Dentsply Sirona SDR, Tokuyama Estellite Bulk-fill Flow). Composite disks with a diameter of 13 mm and a thickness of 1.5 mm were prepared for each subgroup using polytetrafluoroethylene molds. After polymerization, both surfaces of the samples were polished. The samples were incubated in distilled water at 37°C for 24 hours. Initial color measurements were taken using a spectrophotometer. The samples were then immersed in the coloring solutions for 72 hours. Color measurements were taken again, and ΔE 00 values were calculated using the CIEDE2000 formula. The data were statistically analyzed using three-way variance analysis (ANOVA) and Tukey HSD multiple comparison tests. Results: A significant difference in ΔE 00 values was observed between the three groups. The values in the SDR group were significantly higher than those of the 3M and Tokuyama groups. In the black tea group, the values were significantly lower than those of the red wine and coffee groups but significantly higher than those of the control group ( p <0.05). In the red wine group, the values were significantly lower than those of the coffee group but significantly higher than in the control group ( p <0.05). The values in the coffee group were significantly higher than those of the control group. Conclusion: Coloring solutions have an effect on the color stability of bulk-fill composites. Comparison of light transmission of multilayered zirconia materials produced using different sintering methods

Aim: In multilayered zirconia blocks, the enamel layer consists of 5Y-TZP, whereas the transition and body layers consist of 3Y-TZP content. Zirconias, which have different compositions, can be sintered quickly and conventionally, but the effect of rapid sintering on light translucency is not fully known. The aim of this study was to investigate the effect of speed-sintering protocols on the light translucency of multilayered monolithic zirconia materials. Methodology: In this study, multilayered blocks underwent a speed sintering protocol: Katana Zirconia YML (Kuraray) and IPS e.max ZirCAD Prime (Ivoclar Vivadent). The samples were designed with a post-sintering diameter of 12 mm and thickness of 1 mm, and they were manufactured using a CAD/CAM milling machine (Onex Dental Coritec 350i). Samples obtained from the blocks were randomly divided into two groups and sintered in two different ways according to the sintering protocol (n=16). After sintering, L*, a*, b* values of all samples were measured with a spectrophotometer, and the measurements were recorded using black and white backgrounds. Translucency (TP) values were analyzed using a two-way analysis of variance (ANOVA) test to examine the effects of zirconia firing. Statistical analysis was performed using IBM SPSS. Results: The light translucency values of the ZirCAD Prime subgroup subjected to speed sintering were higher than those of the other groups. In the Katana YML group, no significant difference ( p =0.161) was observed between the subgroups that sintered rapidly and conventionally. Conclusion: The differences in light translucency values may be attributed to variations in the amount and thickness of 5Y-TZP zirconia in the enamel layer. The light translucency of the Katana Zirconia YML block was not affected by the sintering protocol, demonstrating that this block can be safely speed-sintered. Evaluation of surface roughness of composites obtained by additive and subtractive manufacturing methods after sandblasting

Aim: The aim of this study was to observe the change in the surface roughness of composite samples produced by different production methods after sandblasting with aluminum oxide particles and to evaluate the effect of this on the composite material. Methodology: The study tested three subtractive method composites (GC Cerasmart [GC], Vita Enamic [VE] and Grandio Voco [GV]) and one additive method composite (3D Formlabs [3D]) were used. Each subtractive composite had 11 samples (14 x 14 x 2 mm) prepared with an Isomet low-speed saw, whereas the additive composite had 11 specimens produced using an SLA-based 3D printer (Form 2; Formlabs Inc). For roughness measurements and sandblasting, the samples were marked with an adhesive circular paper. Surface roughness was measured using a MarSurf SF1 profilometer, with three readings per sample. Sandblasting with aluminum oxide particles (50 µm) was performed using an Aquacare twin in-mouth sandblaster, and surface roughness measurements were repeated. Statistical analysis was performed using one-way analysis of variance (ANOVA) via SPSS 26, with significance set at p =0.05. Results: Among the composite samples, Vita Enamic (VE) had the highest average roughness value, and 3D Formlabs (3D) had the lowest. VE had a mean roughness value of 0.4575 µm, GC had a mean roughness value of 0.4627 µm, GV had a mean roughness value of 0.665 µm and D had a mean roughness value of 0.4575 µm. All groups were compared pairwise with each other. A significant difference was found between the GV-D, VE-D, GC-GV, GC-VE, and GC-D group pairings ( p <0.05). There was no significant difference between the GV-VE and GC-D group pairings ( p >0.05). Conclusion: Sandblasting with aluminum oxide was observed to increase the roughness of the materials, and the roughness varied depending on the nature of the material. Evaluation of staining susceptibility of teeth after office bleaching

Aim: The aim of this study was to evaluate the discoloration effect of different beverages on previously office-bleached teeth. Methodology: Forty upper incisor teeth with indication for extraction due to periodontal reasons were used in this study. Teeth with discoloration, fractures, caries, or restorations on the crown surface were excluded. Before applying the bleaching agent to the enamel surface, the teeth were embedded into acrylic resin. Two office bleaching sessions were applied. Teeth were randomly divided into five experimental groups (n=8): tea, coffee, cola, and milk, with artificial saliva as the control. The teeth were soaked in beverages for 5 minutes a day for 1 month and were kept in artificial saliva for the rest of the time. Color measurements were taken using a spectrophotometer and the L, C, h values were recorded. Initial color measurements were taken after bleaching sessions. Color measurements were repeated for each beverage after soaking for 24 hours, 72 hours, 1 week, and 1 month and color changes (ΔE 00 ) were calculated using the CIEDE2000 formula. Data were analyzed by two-way repeated analysis of variance using the IBM SPSS 25.0 program ( p <0.05). Results: Teeth kept in coffee and tea revealed significantly different mean ΔE 00 values in the comparisons between the 1st month and 24th hour and between the 72nd hour and 1st week ( p <0.001, p <0.001, and p =0.002, respectively). During inter- and intragroup comparisons of the milk and cola groups, no significant differences were detected at examination intervals ( p >0.05). In the 1st month measurements, the mean ΔE 00 values obtained from samples soaked in coffee and tea were statistically higher than the artificial saliva group ( p =0.006 and p =0.007). The mean ΔE 00 values obtained at the 24th and 72nd hour and at the 1st week did not differ significantly among the tested beverages ( p >0.05). The effect of different materials and multi-unit angles in stress distribution on implant-supported fixed bridges: A finite element method study

Aim: In implant-supported restorations, various abutment angulations can be used. These angles are determined based on factors such as the structure of the jawbone, the position of the implants, and the design of the restoration. The aim of this study is to evaluate the effect of multiunit abutment angles on stress patterns of the implant and the peri-implant tissue. Methodology: In this finite element analysis study, three different three-dimensional models of maxillary 3-unit fixed implant-supported prostheses were examined. A jawbone was created with jaw components in SOLIDWORKS. Subsequently implant models obtained from an implant company(Bilimplant®) were placed in the area of the maxillary premolar, and the other implant were placed in the area of the maxillary first molar. Multiunit abutments were used at three different angles (0°, 15°, 30°). Two different restorative materials (Co-Cr feldspathic and zirconia feldspathic) were designed in EXOCAD software for the fixed implant-supported restoration. 150 newtons at a 30-degree angled oblique force was applied to the implants. Subsequently, stress distribution was evaluated with finite element analysis and performed using the ABAQUS software and analyzed with Von mises stress distribution. Results: With the increase in the multiunit angle in implant-supported prostheses, it has been observed that the stress values applied to the implant components and cortical bone increase, while the stress applied to the prosthesis decreases. The highest stress values on implant components and cortical bone were detected in the zirconia framework models. Conclusion: This study evaluates the impact of multiunit abutment angles on implant-supported restorations and examines the stress patterns of implants and peri-implant tissues. The study shows that increasing the multiunit angle in implant-supported prosthesis increases stress values on implant components and cortical bone while decreasing stress applied to the prosthesis.

The effect of different implant-bone levels on stress distribution in three-unit monolithic zirconia restorations: A 3D finite element analysis

Aim: The aim of this study is to investigate the stress distribution in implant-supported three-unit monolithic zirconia restorations according to different bone levels at which implants are placed in the mandible, using three-dimensional finite element analysis. Methodology: For this study, mucosal, trabecular, and cortical bone pieces were designed using the SolidWorks program to simulate an edentulous mandibular jawbone. Two implants were positioned in areas numbered 44 and 46 with different subcrestal placements (0 mm, 0.5 mm, 1.0 mm, 1.5 mm, 2.0 mm). In each model, dental implants, abutments (TiBase), monolithic zirconia crowns, abutment screws, and bone tissues were simulated. The obtained data were exported as Standard Tesselation Language (STL) files and imported into the Exocad program for the design of prosthetic restorations and custom abutments. The STL files were then imported into the Geomagic Design X program to merge the implants, abutments, and restorations on the digital model. The final models were saved as STP files and transferred to the ABAQUS software for finite element analysis. Subsequently, an oblique load of 150 N in the buccolingual direction was applied to the occlusal table for 1000 cycles, and the mandibular jawbone was fully constrained (encastre) in all directions (both movement and rotation). Stress distribution values were recorded in von Mises (vMS) units, and fatigue performances were measured using the Seegers method. Results: As the implant bone levels increased, the vMS increased on the prosthetic restoration. Specifically, vMS increased on the abutment of implant 44 but decreased on implant 46. Fatigue performance worsened on the abutment of implant 44 but improved on the abutment, and screws of implant 46. Abutment screws proved most durable, with abutments experiencing early failures. Conclusion: According to the results of the study, it has been observed that the implant bone level affects the stresses occurring in prosthetic restoration. Examination of stress distributions on Class V restorations using nanohybrid resin composite: A 3D finite element stress analysis

Aim: The aim of this study was to investigate and compare the distribution of stress zones in composite resins and dental tissues applied to different-sized Class V cavities in the cervical region of the tooth as a result of the forces acting on the tooth in a computer environment using the finite element method. Methodology: The extracted lower first molar tooth was scanned and recorded with a dental tomography device. Images were obtained as DICOM files, and these files were imported into the Mimics 12.00 program. In this program, a 3D image of the tooth was created and converted to Standard Tessellation Language (STL) files. The obtained STL files were transferred to the Geomagic Design X program, and Standard for the Exchange of Product Model Data (STP) files were created. The SolidWorks program was used to create different sizes of Class V cavity preparation, restorative material layers, adhesive material layers, and periodontal tissues. Finally, a finite element (FE) model was created in the ABAQUS program, and stress distributions were analyzed. Results: When the depth was kept constant and the width was increased, the amount of stress on the enamel tissue decreased, while the amount of stress on all other tissues and the restorative material increased. When the width was kept constant and the depth was increased, the amount of stress on the restorative material, adhesive layer, and cortical bone increased, while the amount of stress on the enamel layer, dentin layer, periodontal ligament, and trabecular bone decreased. Conclusion: Based on the values found in the study, the model that produced the least stress on the restorative material was the model with the least depth and width. However, the stress accumulation in the enamel tissue was higher in this model compared to the other models. As a result, the lifespan of restorative materials may be longer in cavities prepared more shallowly and narrowly during the preparation phase. Examination of stress distribution on Class I cavity restored using amalgam and bulk-fill composite: A 3D finite element stress analysis

Aim: The aim of this study was to examine the stress distribution on the tooth structure, restorative materials, and periodontal tissues prepared in a computer environment as a result of the forces acting on the mandibular premolar tooth surface, using the finite element stress analysis method. Methodology: The extracted mandibular premolar tooth was scanned with a dental tomography device and recorded. Images were obtained as digital imaging and communications in medicine (DICOM) files, which were transferred to the Mimics 12.00 program. This program was used to create a 3D image of the tooth, which was converted into a Standard Tessellation Language (STL) file. The resulting STL files were transferred to the Geomagic Design X program, some necessary adjustments, such as smoothing, were made, and Standard for the Exchange of Product Model Data (STP) files were created. The STP files were transferred to the SolidWorks program and Class I cavity preparation, restorative material layers, adhesive material layers, and periodontal tissues were created. Finally, the finite element (FE) model was created in the ABAQUS program, and the stress distributions were analyzed. Results: In the model restored with bulk-fill, compared to the model restored with amalgam, the amount of stress on dentin was found to be higher, while the amount of stress on cortical bone, trabecular bone, periodontal ligament, and enamel was found to be lower. In both models restored with bulk-fill composite and amalgam, the structure exposed to the most stress was the enamel layer, while the structure exposed to the least stress was the periodontal ligament. The amount of stress on the amalgam material was found to be higher than the amount of stress on the bulk-fill composite material. Conclusion: Amalgam and bulk-fill composite materials are frequently used today in posterior teeth with large cavities. As a result of our study, bulk-fill composite material caused less stress distribution than amalgam in all tissues except dentin tissue. Investigation of the effects of various restorative materials on stress distribution in a zirconia abutment in the anterior region: A finite element analysis

Aim: The aim of this study was to investigate the effect of using aesthetic restorations and abutments in cases where implants will be applied in the anterior region on the stress distribution on implant components and surrounding biological tissues. Methodology: The data was created in the SolidWorks program, with cortical bone approximately 2 mm thick and trabecular bone approximately 15 mm thick. An implant obtained from a dental implant company, with a length of 12 mm and a diameter of 3.7 mm, was placed into the created data. The obtained data was exported as the Standard Tessellation Language (STL) file and imported into the EXOCAD program. A custom abutment and a prosthetic restoration with central incisor morphology were designed within the data, and all data were exported in STL format and re-imported into SolidWorks. Cement data with a thickness of 30 μm was created, and then all parts were assembled appropriately. The assembled data was converted to Standard for the Exchange of Product Model Data (STP) format and transferred to the ABAQUS program. In ABAQUS, zirconia was assigned to the custom abutments, dual-cure resin cement to the cement, and three different all-ceramic materials, MZ, zirconia-reinforced lithium silicate (ZLS), and lithium disilicate glass ceramics (LS), to the restoration material. A load of 150 N was applied obliquely at a 30º angle to the palatoincisal corner, and the resulting values were recorded as von Mises stress (VMS) values. Results: In each of the three restorations, the highest von Mises stress value occurred around the abutment, while the lowest stress value was observed in the trabecular bone. The stress value around the restoration was highest in monolithic zirconia. The highest stress value around the abutment was observed in lithium disilicate. Conclusion: In cortical and trabecular bone, similar values were found across all three restorations.
